Fall Sports Parent Meeting-HS Gym 7:00

MCS Parents,

If your child is planning on playing a fall sport: HS Boys & HS Girls Soccer, JR H & HS Volleyball or  JR H & HS Cross Country,  please plan on attending the meeting onThursday, August 3rd at 7:00pm in the HS Gym these are the forms that you will need to fill out:

If your child is in need of a physical you, your child and  your physican will need to fill out the following forms:

Physical/Concussion Form/Avita Consent to Treat

* Please note if your child does not have a current physical on file they will not be permitted to practice or play until it is turned in*

Athletic Fee:

7th-8th:  $75

9th-12th: $115

*Family Max is $285*

Athletic Passes:

Adult: $75

Family: $175

*Please Make Checks Payable to MCS*

*If you are planning on transporting any of the players to the game the Secondary Office will need a current copy of your car insurance*
