Avita is providing FREE Sports Physicals

FREE Sports Physicals for the upcoming 2017-18 Academic Year will be held at Avita Ontario (Richland Mall) on Wednesday, May 10th from 5:00-8:00pm. Students entering grades 7-12 planning to participate in athletics in the upcoming school year are encouraged to attend this FREE EVENT.  Please have your athlete bring a COMPLETED sports physical packet to the evaluation on May 10th.  Important signature lines have been highlighted in yellow for easy identification.  PLEASE NOTE: The signature line highlighted in yellow on page 4 will be completed on the day of the evaluation by the physician.

IMPORTANT: Athletes MUST have completed sports physical paperwork submitted to the Athletic Department BEFORE they are permitted to  participate in any athletic practices and games/meets.

Please have your student stop by the Secondary Office or Central Office to pick up a Sports Physical packet.  You may print your own forms by clicking on the following: PHYSICAL PACKET

The following MCS sports will require a Sports Physical for participation (JH = grades 7-8; HS - grades 9-12):
