Elementary Box Top Collection Form

Instructions:

*Look for Box Tops coupons on products you use every day,

*Make sure they have not expired,

*Neatly cut out the coupons on the dotted lines,

*TAPE (glue will not hold) them to the collection sheet, and

*Be sure to put your name, grade, and teacher’s name on the bottom of the sheet before

  turning it into your teacher or the office.  Once you have filled out a collection sheet simply

  request another from your teacher or the office.

*All Bonus Box Tops coupons should be placed in a bag or envelope labeled with your child’s name,

  grade and teacher’s name – please do not attach them to the collection sheet.
